Open up the excel file and fill out the form using a 0 – 10 scale. If it does not apply to you, leave it blank. 10 is the worst you can possibly imagine. 1 is every so often. Once finished, save it to your computer, attach the document to an email and send to Dr. Bob at drbobapol@gmail.com, 2 days prior to your next nutrition visit.
